Transitional Funding Program

AVAILABILITY AND MAXIMUM FUNDING
Disabled Resources Center has a grant, allowing funding upon completion and approval
of application. ***No Cash Loans are available***
TRANSITION FUNDING AVAILABLE FROM THE FOLLOWING FACILITIES
Funds available to enable consumer to transition only from one of the following types of
facilities:

• Licensed Skilled Nursing Facility (SNF);
• Intermediate Care Facility for the developmentally disabled;
• State Hospital for the mentally ill;
• Developmental Care Center; or
• California Veterans Home.

ALLOWABLE PURPOSES FOR USE OF TRANSITION FUNDING MAY
INCLUDE, BUT MAY NOT BE LIMITED TO THE FOLLOWING:
• 1st month’s rent (no last month or deposits);
• Initial stock of personal items, clothing, food;
• Personal Assistant issues not covered by IHSS;
• Initial stock of household necessities;
• Moving Expenses
• Retrieval of items in storage;
• 1st month’s utilities ( no refundable deposits);
• Shipping charges not to exceed $50.00;
• Assisted Technology Items;
• Occupational therapist assessments (outside sources only);
• Physical therapist assessments (outside sources only); and
• Assisted technology assessments (outside sources only).

FUNDS INTENDED FOR:

Funding intended to provide a “link” of assistance in the transition process where none is available from another source. No other funding, including medi-cal is available to the consumer. This funding is a “funding of last resort.” Exception: If other funding opportunities for AT have been exhausted and the transition of the consumer is dependent only on the resolution of a medi-cal appeal (following medi-cal’s denial) transitional funding may be accessed for the purchase.

This program is intended as one-time only per consumer.
