Aging and Disability Resource Connection (ADRC)
MISSION STATEMENT
The mission of the Emerging Aging Disability Resource Connection is to provide a seamless No Wrong Door Service policy and strategy so both the Senior and Disability Communities can easily negotiate the complex network of public and private, social, medical, transportation and other services. The strategy of efficient communication between and among service providers defines the policy of No Wrong Door for consumers needing help where people can link head and heart into an integrated, intelligent solution.
ARE YOU WONDERING HOW THE AGING AND DISABILITY RESOURCE CONNECTION (ADRC) CAN HELP YOUR CLIENT, CONSUMER, OR FAMILY MEMBER?
ADRCs offer 4 core services as explained below. ADRCs are a community collaboration of service providers and other partners working together to improve the ways in which consumers access and receive services. Consumers of any age, disability, or income level can contact any one ADRC partner organization to access a wide array of Long-Term Services and Support options offered by ADRC partners in the community.
SERVICES
ENHANCED INFORMATION & REFERRAL SERVICES
Speak with a live person who will provide comprehensive resources, information, follow-ups, and communication between agencies.
OPTIONS COUNSELING
Assist in identifying goals and needs through person-centered counseling and coordination access to public and private-funded long-term services and community support.
SHORT-TERM SERVICE COORDINATION
Expedited access to services and support for individuals at risk of institutionalization, generally for 90 days or less, until a longer-term plan is in place.
TRANSITION SERVICES
With a person-centered model, we help persons with disabilities and persons 60 and older move from restrictive to less-restrictive environments. For example, transitions can happen to/from hospitals, nursing homes, or other institutions back into the community. The goal is to work with the individual and their family to make decisions that match the preferences, values, and financial living standards.
