50th ANNUAL AWARDS DINNER NOMINATION FORM
Disabled Resources Center, Inc. (DRC) hosts an Annual Awards Recognition Event, where we come together in celebration of another successful year to honor individuals who stand out for the commitment in serving the needs of people with disabilities. This year’s Annual Awards Recognition Event is scheduled for July 23, 2026. DRC has designated four areas highlighting our commitment to the community.
Currently, we are seeking nominations of people who, such as yourself, have made our community a better place in which we live. Please take some time to reflect on individuals who, in your opinion, deserve to be honored at DRC’s 50th Annual Awards Recognition Event and complete this Nomination Form. Please return your nominations by April 30, 2026. Nominations will be reviewed by the Awards Event Committee and selected by DRC’S Board of Directors.
Jerry Stein Independent Living Award: Honors a person who has demonstrated extraordinary achievement in his/her professional career or volunteerism. This award also promotes awareness of the achievements of people with disabilities.
Civic Leadership Award: Honors a person who tirelessly helps people with disabilities achieve a higher level of independence through his/her efforts in advocating system changes of empowerment for people with disabilities.
Community Service Award: Honors a person committing time and resources toward empowering people with disabilities in our communities.
Outstanding Military Veteran Scholarship: Awarded to a military veteran who is either currently enrolled in or is planning on enrolling in a program of post-secondary education.
